Requesting Grand Junction Fire Dept. Incident Reports

General Information

Fee: $1.25 per report requested, up to 6 pages, and $2.50 for larger reports will be charged. All related fees must be paid before a request can be released. Make check
or money order payable to the Grand Junction Fire Department. Request should be made at GJFD's Fire Station 1, 330 South 6th Street, Grand Junction, CO 81501.

Medical Incident Reports

Medical Record requests not made in person require all signatures to be notarized.

Medical record requests. 

• Please provide a completed and signed record request form and a copy of your photo ID.
• Parents may request information on behalf of their minor children.  Please supply a completed and signed record request form and a copy of your photo ID.
  
• A request on behalf of another adult can be processed with a notarized statement of consent from the patient to the requesting party, a completed and signed record request and a copy of the photo ID of the requesting party.
• Attorney request for patient records can be processed with a notarized statement of consent from the patient to the Attorney and a completed and signed record request
